New Patient Information

Welcome to the Siletz Community Health Clinic

The Siletz Community Health Clinic serves Indians and Non-Indians. Eligible Native Americans can receive most SCHC services at no cost after insurance.  Non-Federally Recognized Indians and Non-Indians are responsible for the cost of services not covered by insurance.


If you are interested in becoming a patient, please submit the following to the Patient Care Coordinator located in the clinic’s reception area: 

· New Patient Registration application

· Photo identification such as your driver’s license or tribal enrollment identification card (this requirement is waived for minors)

· Birth certificate

· Social Security card

· Insurance documentation

You will need to check in at SCHC Reception 15 minutes prior to appointment.
