The Siletz Tribal Health Care Program is the delivery of comprehensive health care to the Siletz Indian People and their dependants residing within the eleven county service area of Western Oregon. Services are provided through a combination of direct care, contracts and referral to appropriate agencies.

Optometry »

Optometry

The Optometry Clinic is open five days a week. Appointments to see an Optometrist are available Monday thru Thursday. Full optometric services, including eye examinations and eyeglasses, are offered to the public. Patients must be registered at the clinic before an appointment can be made. Eyeglasses and contact lenses, which provided by contract optical labs, are available to patients at cost and must be prepaid. Any patient responsibility must be paid BEFORE the glasses or contacts are ordered. Patients wishing to have their glasses or contacts mailed will be required to pay the shipping fee in advance.

Optometry prescriptions written by a doctor outside the Siletz Clinic will be honored until the expiration date of the prescription. The Siletz Clinic is not responsible for any returns due to an incorrect outside prescription.

The Optometry services are provided by a credentialed Optometrist.
