The Siletz Tribal Health Care Program is the delivery of comprehensive health care to the Siletz Indian People and their dependants residing within the eleven county service area of Western Oregon. Services are provided through a combination of direct care, contracts and referral to appropriate agencies.

First time patients must arrive an hour early to complete the registration process. Please bring your Social Security Card, information regarding your health insurance, and if applicable, bring proof that you are a tribal descendent with a federally recognized tribe and a copy of your birth certificate.
The Siletz Clinic is not a walk-in clinic, however, if you feel you need to be seen by a provider on the same day, please call for an appointment and we will do our best to meet your urgent care needs.
Promptness is appreciated! Patients who are 10 minutes or more late will be asked to reschedule their appointment for another day.
Broken appointments or late cancellations interfere with smooth clinic operations and pose an inconvenience to other patients. If you find you are unable to keep your appointment, please call as soon as you know so we can work another patient into the schedule.
