The Siletz Tribal Health Care Program is the delivery of comprehensive health care to the Siletz Indian People and their dependants residing within the eleven county service area of Western Oregon. Services are provided through a combination of direct care, contracts and referral to appropriate agencies.

Out of area funding is now available through June 30th, 2010. If you are eligble for Out of Area funding and need medical, vision, dental, or hearing services, please call Siletz Contract Health @ 1-800-628-5720 and get an authorization number

During regular business hours, call the Siletz Clinic. After regular business hours, on weekends or holidays please call the above number. You will be directed to the answering service, who will alert the physician on call.
