The Siletz Tribal Health Care Program is the delivery of comprehensive health care to the Siletz Indian People and their dependants residing within the eleven county service area of Western Oregon. Services are provided through a combination of direct care, contracts and referral to appropriate agencies.

Diabetes Program

Diabetes Program Assistant:  Connie Anicker

Diabetes Education:
Individual or family appointments are available with our Certified Diabetes Educator (CED). A series of visits is recommended for those newly diagnosed with diabetes, and once each year for others. Available for clinic patients and tribal members with diabetes.

Diabetes Talking Circle:
An educational and support group that meets the third Thursday of every month over the lunch hour. Free lunch and educational presentation. Available to those with diabetes, their families and interested others.

Exercise Opportunities:
Classes and opportunities throughout the year are available for all community members.
