The incumbent shall be sensitive to the diversity of Native cultures as well as have knowledge of case management and the ability to provide referral resources.
Associate’s degree in Human Services or related field and one year experience in determining eligibility, case management, employment/vocational counseling, or general counseling –OR- an equivalent combination of education and/or experience. Knowledge of: Tribal and other services available to assist the person in wellness. Substance use disorders, mental illness, socioeconomic and cultural factors.
Ability to: Work with a wide variety of people, and communicate effectively with program staff, clients, and the community. Demonstrate cultural competency in direct service, within the organization, and in program planning and development. Ability to prioritize both independently and as a member of a multidisciplinary team. Other: Must possess First Aid and CPR certification, or must acquire within 30-days of hire. Must possess a valid Oregon State driver’s license, insurance and be eligible to drive a GSA vehicle.
Provide skill building for independent living, physical, and recreational activities. Participate with clinical staff in making referrals to tribal and other community resources leading to self-sufficiency; Prepare and complete program reports; Update records to document client services delivered; Maintain up-to-date client records; Gather documents and complete intakes for enrollees in Targeted Case Management; Maintain intake packets, including updating forms and releases within software program; Ensuring documentation in files/computer is up to date and accurate; Collaborate with multiple departments to track needs of individuals coming out of incarceration; Contact facilities to determine service availability; Develop and maintain referral tracking process including completion and outcome of referral; Assist clients with over- the- phone screening processes; Arrange and assist with transportation as needed; Gathering reports from partners to forward to other partners (e.g. 477 Self Sufficiency, Vocational Rehab, courts, probation, ICW, ODHS); Attend monthly meetings with other partners (e.g. 477 Self Sufficiency, Vocational Rehab, courts, probation, ICW, DHS) as requested; Review and understand insurance information provided by the in-patient admissions office, clarifying which benefits are available to the client; Develop and utilize comprehensive list of community resources including tribal, county, state and charitable/private resources. Serve as the client advocate to get needs met related to intake and assessment; Work collaboratively with partners to verify the client’s needs are being met; Provide appropriate communications with tribal and community partners or other referral agencies regarding client compliance; Other duties as assigned
All new employees will serve a six-month probationary period to enable the General Manger to determine their suitability as a tribal employee. After the six-month probation the employee will be evaluated of his/her performance and at that time, it will be, decided if the employee is entitled to a regular employee status.

Conditions of Employment
