Eligibility

All services are free and available to enrolled Siletz tribal members.

Services

The Community Health Department aims to provide quality outreach health care services and health promotion/disease prevention services to Native Americans within our community through the use of well-trained representatives. We provide confidential and compassionate health services. We are here to assist Siletz tribal members make healthier decisions to improve their quality of life.

Community Health Advocates

Community Health Advocates provide a variety of assistance services.

Harm Reduction Services

Includes Tobacco Prevention, Early Intervention, and Peer Support.

Tribal Non-Emergency Medical Transportation

Transportation can be provided as a last resort to eligible Tribal members.

CARE Program

For education and advocacy services to victims of domestic violence, sexual assault, dating violence and stalking.
